Worked example: a near miss told in plain words
A delivery driver describes a near miss at the loading dock.
Step 1: Told in their own words
The driver tells the assistant what happened, the way they would tell a colleague.
Step 2: Drafted for them
The assistant drafts the report and suggests the event type and severity, each marked as suggested.
Step 3: Only the gaps are asked
The assistant asks for the dock number and the time, the two things that were missing.
Step 4: A person accepts it
The driver checks the draft and submits it as theirs. Nothing reaches your records until then.
